DIGOXIN ECG CHANGES DIGITALIS TOXICITY-DIGITALIS EFFECT
Digoxin ecg changes-Digitalis refers to class of radioactive drugs called glycosides which having both mechanical and electrical effect on the heart,Most commonly used digitalis preparation in current practise is digoxin. It have positive inotropic and negative chronotropic effect in our heart
Effect of digoxin |
Since a number of more efficacious and safer medications have become available, the use of digitalis is mostly limited to patients with AF,systolic congestive heart failure and hypotension who cannot tolerate beta blockers or calcium channel blockers, concentration of digoxin in the serum can be measured by radioimmunoassay.Therapeutic concentration range is from 0.5-2 ng/mL
Concentration exceeding 2.0 are associated with high incidence of digitalis toxicity.Digitalis toxicity is different from digitalis effect.Toxicity is the arrhythmias and conduction disturbances produced by this drug-Digitalis effect is the thumb printing or reverse tick mark sign of ST-T complex in electrocardiogram.
Digitalis effect in ecg |
Factors predisposing digoxin toxicity are electrolyte disturbances, advancedage,HCM,M I,Renal insufficiency, chronic lung disease, heart failure caused by amyloidosis, wpw syndrome and drug to drug interactions.symptoms of digoxin toxicity are weakness, anorexia, nausea, vomiting.Prevention of digoxin toxicity include before starting the usage of digoxin indication should be carefully reviewed.
Digitalis toxicity |
Early signs of digoxin toxicity should be carefully checked.Digoxin dosage should be lowered when starting the medications which have an effect of increase of digoxin levels.Treatment of digitalis toxicity depends upon particular arrhythmia.
With minor arrhythmia discontinuation of digitalis and in serious arrhythmias suppression with intravenous drug such as lidocaine is needed. For tachycardia, potassium supplements should be given to raise the serum potassium level.
Patients with complete heart block from digitalis toxicity may require a temporary pacemaker.Patients with potentially lethal overdose can be treated with a special digitalis binding antibody given intravenously.
