HEART RATE CALCULATION IN ECG-HEART AND TECHY


First of all we are discussing about rate of e c g paper,Before calculating heart rate from an e c g you have to know what is meant by standardisation in an e c g paper

X axis of the paper denotes the time and Y axis denotes the amplitude.One large box in an e c g paper contain 25 small boxes totally that is 5 small boxes in x and 5 small boxes in y axis.

In x axis duration of one small box is 0.04 second ,so duration of 5 large box is 0.2 second.In Y axis amplitude of one small box is 0.1 millivolt and 5 large box is 0.5 millivolt,first step while reading e c g is to look for whether standardization is properly done.

This is the standardization mark in the ecg.We have to look at this vertical mark in the e c g  and see the mark exactly covers two big squares.Standard calibration is 25 mm/second,0.1millivolt per millimeter.

For calculating heart rate there are different calculation methods are used for regular and irregular rhythms

If the rhythm is regular 300 method is used,very fast rhythm 1500 rule is used.In both of these methods number of large or small squares in the rr interval that is one beat is divided from 300 or 1500.

1500/number of small squares in r-r interval(for fast rhythms this formula is used to calculate the heart rate)

For very slow or irregular rhythm number of r waves in the rhythm strip is multiplied with 6 that is average rate over 10 second period.10 multiplied with 6 we will get 60 second that is one minute to give average beats per minute.

Normal heart rate is 60 to 100 beats per minute for adults,less than 60 is brady and more than 100 is tachycardia but for newborns the heart rate is in the range of 110 to 150 beats per minute

This rate will be come into the 60 to 100 beats per minute when they have 6 years old.

If the patient have complete heart block we have to calculate the atrial and ventricular rate separately

Most of the time the machine will calculate the heart rate accurately automatically by itself.But sometimes for example if the patient have abnormal qrs and t complexes or missed qrs complex or reduced amplitude of the qrs complex may result in mistake in the calculation of heart rate.
